What is the DHI Hair Transplantation Process exactly?

The DHI method (Direct Hair Implantation) uses the so-called Choi Implant Pen: a precise instrument that extracts hair follicles and immediately implants them into the recipient area — without prior incision of channels. It sounds technical, but it makes a crucial difference: The follicles are exposed to stress for a shorter time, which increases the survival rate to up to 95–97%.

1. Initial Consultation and Hair Analysis

Everything begins with a digital hair analysis. Our specialists assess the donor density in the back of the head, the depth of the receding hairline according to the Norwood scale (Stage I–VII), and the desired hairline. This consultation lasts 30–45 minutes and is free for Hair360 patients.

Our Tip: Send three recent photos (top view, side, back of the head) — this way we can already provide a realistic prognosis before the consultation.

2. Surgical Planning and Hairline Design

On the day of the operation, the doctor outlines the new hairline. This takes 20 minutes and is crucial: A natural hairline follows slight irregularities; it never looks perfectly symmetrical. Only agree to the design when you are sure — changes are easily possible before the surgery.

3. Anesthesia of the Donor and Recipient Area

Local anesthesia via injection. The first seconds are uncomfortable, after that you feel nothing. Many patients fall asleep during the surgery. Newer clinics use the “No-Touch” pen, which significantly reduces the pressure of the syringe — Hair360 uses this system as standard.

4. Extraction of Hair Follicles

With the Choi Pen or a micro punch, individual follicular units (1–4 hairs) are extracted from the donor area. In a typical session with 3,000–4,000 grafts, this step takes 2–3 hours. The extraction sites heal invisibly within a few days.

5. Preparation and Quality Control of the Grafts

The extracted grafts are examined under a microscope and stored in a special nutrient solution (Hypothermosol or similar). Damaged follicles are sorted out. This phase is crucial for the later survival rate — a step that cheaper clinics often skip.

6. Implantation with the Choi Pen

Now comes the heart of the DHI Hair Transplantation process: The doctor implants each graft individually, precisely determining the angle (35–45°), depth, and direction.
